To submit a request, complete the following steps. Open the Ship Sanitation Inspection Request form at https://safeport-dev-public-hc-sc. powerappsportals.com. Complete the Shipping Agency Information. Required fields are indicated by an asterisk beside the field. Name of Shipping Agent, Company. Office. Telephone number. E-mail address. Click Next. Complete the Invoicing Information. At the top of the page, you can select if you are Canadian Coast Guard or not. The Coast Guard selection affects the fields on this page. Complete the details as required. Required fields are indicated by an asterisk. Include the invoicing name, country, province/state, city, address, and postal code. If you are a member of the Canadian Coast Guard, you will need other information including the Interdepartmental Settlement Organization number and the Reference Number. Click Next. Complete The Vessel Information. Include the International Maritime Organization (IMO) number. Ship Name. Registry (Flag) Ship owner. Ship particulars that indicate the information about the ship. Existing ship sanitation certificate. Click Next. Complete the Service Information to indicate where the ship will be for the inspection time.
